FOOD & BEVERAGES

• Domestic Food Facility Registration – $375/Bi-annual fee
• Foreign Food Facility Registration & U.S. Agent Services – $900/Bi-annual fee

DIETARY SUPPLEMENTS

• Domestic Food Facility Registration – $375/Bi-annual fee*
• Foreign Food Facility Registration & U.S. Agent Services – $900/Bi-annual fee*

*Facilities that manufacture, process, pack or hold dietary supplements also register as domestic or foreign food facilities. There is no separate dietary supplement facility registration. 

MEDICAL DEVICES

• Domestic Medical Device Establishment Registration – $1,100/yearly fee*
• Foreign Medical Device Establishment Registration & U.S. Agent Services – $1,100/yearly fee*

*With the passage of the Medical Device User Fee Amendments of 2012 (MDUFA III), all medical device establishments must pay the FDA registration fee, regardless of establishment type or activities conducted. This registration fee will be added to the above described service fees at checkout, and must be paid in advance of the rendition of services by FDA Solutions Group. Current fees can be found on the medical device establishment registration pages.

COSMETICS

• Domestic Cosmetic Facility Registration - $975/Bi-annual fee
• Foreign Cosmetic Facility Registration & U.S. Agent Services - $1,275/Bi-annual fee
• Cosmetic Product Listing - starts at $4,500/yearly fee. Pricing dependent on number of products.
